On August 20, 2026, JAMA Network Open published new findings regarding a culturally adapted Mediterranean dietary pattern. The DELISH randomized clinical trial compared this program against a control intervention for two years. The control group provided participants with free access to WeightWatchers workshops and digital tools. Researchers enrolled 360 adults with obesity to measure how a regional approach impacted long-term nutrition.
The Mediterranean diet is heavily researched for its potential health benefits. However, enforcing a rigid set of foreign recipes can make long-term compliance incredibly difficult. The Med-South intervention solved this by adapting the dietary pattern to regional tastes in the Southeastern United States. It emphasized vegetable oils alongside nuts and seeds.
The plan also prioritized fish, whole grains and fruit while remaining culturally familiar. Non-starchy vegetables were heavily promoted as foundational components of the daily eating pattern. At the same time, the program guided participants to reduce their intake of refined carbohydrates and highly processed foods.
This strategy shifts the metabolic focus away from pure calorie restriction toward overall food quality. Med-South participants reported consuming a higher proportion of energy from beneficial fats. These included monounsaturated fat and polyunsaturated fat, alongside a lower proportion of daily carbohydrates. Registered dietitian Emily Case noted that the Mediterranean pattern provides a clear blueprint for healthy food choices.
She also cautioned that healthy foods can still be overeaten, making portion awareness highly relevant. For years, I watched smart, capable people blame themselves when standard diet advice failed them. They would cut calories drastically, run themselves into the ground and inevitably regain the weight. It was heartbreaking to see.

Months five through 12 focused on weight loss while maintaining the new eating pattern. Finally, months 13 through 24 transitioned into a dedicated maintenance phase.
This structured approach helps adults build a sustainable weight-management system without relying on extreme initial restrictions. When new nutritional research is published, the mainstream media often frames the results as a competition. Some coverage might suggest that the Med-South approach completely outclassed its commercial competitor. The actual clinical data tells a much more measured and realistic story.
The trial showed that both the culturally adapted program and the WeightWatchers control produced statistically similar weight loss. Neither intervention resulted in a dramatic physical transformation. In the primary analysis, adjusted mean weight change was a 3.40 percent loss for Med-South. The WeightWatchers control group saw a 3.51 percent loss over the same period.
Both groups ultimately fell below the 5 percent average weight-loss level that is commonly considered clinically significant. Approximately 33 percent of the Med-South group achieved at least a 5 percent reduction. In the control group, 36 percent of participants reached that exact same benchmark. The study authors were careful to limit their interpretation of these results.
They stated that behavioral programs are reasonable for people pursuing modest losses. They suggested that individuals seeking substantial weight loss may need options like bariatric surgery or GLP-1 agonist treatment. Substantial weight loss was defined in the study discussion as more than 10 percent of body weight. The findings confirm that managing your weight depends on practical nutrition rather than raw willpower.
The strongest defensible message is not that the Med-South program simply beats WeightWatchers. Instead, it shows that a culturally adapted eating pattern can match a commercial program’s modest outcomes. Furthermore, this flexible approach generated larger and more sustained improvements in reported diet quality. Adults looking to compare popular eating approaches should recognize the value of these qualitative dietary improvements.
The DELISH trial recruited an excellent demographic for studying midlife metabolic health. Participants had a mean age of 54.7 years and a median baseline body-mass index of 35.1. The study assigned 182 people to the Med-South program and 178 people to the WeightWatchers control group. Returning participants in both groups achieved average weight reductions of about 10 pounds over 24 months.
MedPage Today translated the adjusted results into average losses of 8.7 pounds for Med-South and 8.8 pounds for WeightWatchers. The timeline of these scale changes revealed an interesting dynamic between the two methods. The WeightWatchers control group lost significantly more weight during the first four months. They held a 3.44 percentage point advantage over the Med-South participants early in the trial.
However, that initial scale advantage was not maintained at the 12-month or 24-month assessments. This highlights the vital difference between short-term dietary changes and long-term behavioral maintenance. The major separation between the two interventions appeared in their nutritional quality scores. At 24 months, the Med-South group improved its Alternative Healthy Eating Index score by 6.57 points.
The control group improved by only 2.95 points on the exact same scale. This produced a statistically significant between-group difference of 3.62 points favoring the Mediterranean approach. The Med-South group also demonstrated greater improvements in DASH and alternate Mediterranean diet scores. Other physiological and behavioral metrics showed parallel changes across both groups.
Blood pressure declined for all participants, with no significant difference between the two interventions at 24 months. Physical activity and depression-score changes also did not differ significantly between the groups. The researchers utilized a skin-carotenoid measure called the Veggie Meter alongside standard food-intake questionnaires. Veggie Meter scores improved in both groups, though the control group saw a larger early increase.
The trial did experience some expected attrition over its 24-month duration. Final in-person measurement attendance was approximately 69 percent for the overall sample. However, researchers utilized electronic health-record weights to raise available weight data to 94 percent. This robust data collection helps validate the program's long-term acceptability among participants.
At 24 months, 94.8 percent of answering Med-South participants said they would recommend their assigned program. In comparison, 68.3 percent of the WeightWatchers control group offered the same recommendation. Diet quality was assessed partly through participant self-report, which can introduce recall and social-desirability bias. Despite this limitation, the diet-quality findings remain meaningful outcomes for evaluating lifestyle interventions.
The cultural adaptation of the design is a massive strength for application in the Southeastern United States. However, it also limits simple one-to-one extrapolation to other global regions. The DELISH trial clearly demonstrates the value of culturally flexible dietary interventions. Thomas Keyserling, one of the study’s authors, stated that incorporating a healthful dietary pattern into weight-loss programs should be a priority.
He noted that such patterns may offer additional chronic-disease-prevention benefits beyond scale changes. However, the trial did not objectively establish that this improved diet quality directly reduced obesity-related illness. Further research is necessary to determine exactly how diet quality affects specific health outcomes in behavioral programs. The study authors also called for new research combining these diet-quality interventions with effective weight-loss medications.
Clinical trials need to evaluate whether healthier eating patterns improve outcomes during active pharmaceutical treatment. Furthermore, researchers must investigate if these foundational habits reduce weight regain after a patient discontinues medication. Until those studies are completed, prioritizing food quality remains a practical step for supporting long-term metabolic health.
